Part two of this series continues the conversation about what a cure for type 1 diabetes could actually look like, from cell replacement and immune therapies to insulin innovation and advanced diabetes technology. The discussion explores islet cell transplants, stem cell-derived beta cells, immune suppression, gene-edited cells, and the challenges of making these approaches safer and more widely available. The expert panel also break down why even the most advanced technology today still falls short of a true cure. A major focus is accessibility, including who could benefit from these therapies, how much they might cost, and the infrastructure needed to deliver them to millions of people with type 1 diabetes. The experts discuss their vision for the future of cure research and how far off it could be.

In this episode, I sit down with leading type 1 diabetes researchers and clinicians to break down the science behind prevention, delaying disease progression, and replacing insulin-producing beta cells. We explore promising therapies like teplizumab, ATG, and verapamil, along with the challenges of immunotherapy, beta cell replacement, and making these treatments accessible to more people. This conversation also tackles why we’ve been hearing “a cure in 5 to 10 years” for so long, what researchers have learned about the root causes of type 1 diabetes, and why there’s more progress happening now than ever before.

When your CGM fails, your pump dies, or you're caught in a situation without access to your tech, do you know what to do? Gary Scheiner and I break down the "back to basics" skills every T1D needs to know from manual carb counting to calculating correction doses without a pump, to safely transitioning from a pump to MDI (and back again). We also cover what belongs in your diabetes emergency go-bag and how to best manage your care in the event of a natural disaster.

A cure for type 1 diabetes has always felt like a distant idea, but for Patti Columbe, it's now her reality. After living with type 1 for 40 years, Patti shares her journey as a participant of the Vertex VX-880 clinical trial, where a lab-grown islet cell transplant has left her insulin-free and functionally cured. She opens up about the emotional challenges of adapting to life without diabetes and why she believes hope is finally becoming reality for the T1D community. This conversation also explores the future of stem cell therapies, immunosuppression, and what it could take to make these treatments available to more people.

Greg Peterson has lived with type 1 diabetes for nearly 70 years and used an implantable insulin pump for more than two decades, even traveling from the U.S. to France to maintain it after the technology was discontinued. He shares how the implantable pump worked, how it dramatically improved his quality of life, and the profound impact it had on managing highs and lows as well as his relationships with loved ones. Two companies are currently working to bring next-generation implantable insulin pumps back. This conversation dives into the history of this remarkable technology, why it disappeared, and what the future could hold for implantable insulin pumps.

How to Eat with Diabetes: Top Carb Counting Tips

Gary Scheiner, CDCES, and I break down why carb counting is one of the trickiest parts of managing type 1 diabetes, and why getting it "close enough" is often more realistic than getting it perfect. We dive into how fat, protein, fiber, and alcohol each affect blood sugar differently, plus why insulin timing matters just as much as the dose. Gary shares practical estimation strategies like the hand and fist method, carb factors, and using AI tools and apps to get quick, accurate counts on the go. We also cover common "kryptonite foods" that trip people up and how to use CGM and pump data to fine-tune your ratios over time.

Katie Beth Hand, patient 9 in the Eledon trial, received an islet cell transplant in January 2026. She is now living a "functionally cured" life, but every 21 days she requires a transfusion of an immunosuppressant called Tegoprubart which stops her body from attacking the newly implanted islet cells. In this conversation, we get into how the trial works, what it feels like to stop taking insulin after years of type 1 diabetes, and the complicated emotions that come with this new normal.

Sending a child to school with type 1 diabetes should be easier in 2026. In this episode I break down why it remains one of the most stressful parts of managing the disease. I sit down with Dr. Taylor Inman, a physician and parent of a child with type 1 diabetes, to unpack the major gaps in school-based diabetes care despite widespread use of CGMs and insulin pumps. We explore how outdated policies, lack of training, and limited support systems are forcing families into constant crisis mode, sometimes even pushing parents out of the workforce. We also dive into her national survey data, revealing how common these challenges are across the U.S. and what needs to change.

More about Justin
Justin was diagnosed with Type 1 Diabetes in May 2021 thanks to the diabetes community on Tik Tok.
Originally, misdiagnosed with type 2 diabetes, Justin posted videos on Tik Tok of his symptoms and what his doctor had him doing at the time - testing blood sugar, taking long acting insulin - when the diabetes community found his videos, they blew up and they told him he had type 1, needed and Endocrinologist, a cgm and a pump. They got him the help he needed and I owes them his life.
