I share my unfiltered review of the twiist insulin pump, diving deeper into my personal experience after two months of real-world use. I go beyond a standard review to break down how features like Apple Watch control, glycemic index-based carb logging, and retroactive carb adjustments performed in my day-to-day life. I also get the highs and lows, from strong glucose control and 87% time in range to frustrations with the Cleo infusion set and managing a tubed system. We also cover CGM integrations and how it felt using Libre 3 Plus with an AID system. This deeper conversation covers everything from training and usability to app design, connectivity, and overall reliability.

$11 Insulin for All? Inside the Nonprofit Rewriting U.S. Drug Prices
Monday Apr 20, 2026
Monday Apr 20, 2026
I sit down with Allan Coukell, Civica's Chief of Government Affairs & Public Policy Officer, to explore how its nonprofit insulin model and California’s CalRx partnership are bringing low-cost insulin—priced around $11 per pen and $30 per vial—to people nationwide, with or without insurance. We break down how pharmacy benefit managers, rebates, and the current drug pricing system have kept insulin prices high, and what’s finally shifting. We also dive into the rise of biosimilar insulin and how this approach could transform drug affordability far beyond diabetes.

Optimizing Mealtime Insulin for Better Post-Meal Glucose - ODT
Monday Mar 30, 2026
Monday Mar 30, 2026
Gary Scheiner, CDCES, and I break down how to improve mealtime insulin dosing and why boluses still matter even with automated insulin delivery systems. We talk through insulin-to-carb ratios, pre-bolus timing, extended boluses, and how different foods impact post-meal blood sugar patterns. Gary shares practical ways to read your CGM data to tell whether highs and lows are caused by timing, dosing, or carb counting challenges. We also cover strategies for handling slow-digesting meals, high-fat foods, and unpredictable eating situations without increasing hypoglycemia risk.

Monday Mar 23, 2026
Monday Mar 23, 2026
I spoke with endocrinologist and diabetes tech expert Dr. David Ahn at the ATTD conference about the biggest breakthroughs shaping the future of diabetes management, including the rapid progress toward fully closed-loop insulin delivery systems. We explored how companies like MiniMed, CamAPS, and Omnipod are advancing automation that reduces carb counting, minimizes manual bolusing, and improves time-in-range outcomes. We also discussed emerging topics such as GLP-1 therapies for type 1 diabetes, AI-powered diabetes tools, and continuous glucose-plus-ketone monitoring aimed at preventing diabetic ketoacidosis. We shared key takeaways from ATTD and what these innovations mean for reducing daily diabetes burden while moving closer to a true artificial pancreas.

Detained by ICE and Denied Insulin for Days - She Shares Her Story
Monday Mar 09, 2026
Monday Mar 09, 2026
I sit down with Hanne Engan to discuss her experience being detained by ICE during what was supposed to be a routine U.S. green card interview. We unpack how immigration enforcement policies intersected with a life-threatening medical crisis when Hana—who has type 1 diabetes—was denied critical insulin and medical care inside an immigration detention center. Through her firsthand account, we explore the realities of visa overstays, ICE detention practices, and the human impact of immigration policy in the U.S. This powerful conversation sheds light on medical neglect in detention facilities and raises urgent questions about immigrant rights, healthcare access, and accountability.

How to Optimize Your Insulin for Better Time in Range - ODT
Monday Mar 02, 2026
Monday Mar 02, 2026
Gary and I break down why insulin doesn’t always work the way we expect—whether it’s absorption issues, site problems, heat exposure, stress, hormones, illness, or exercise. We talk about how automated insulin delivery systems are only as good as the insulin actually getting into your body, and why fixing efficacy often matters more than changing your settings. Gary shares practical strategies for improving absorption, rotating sites effectively, and troubleshooting those “stubborn” highs that won’t come down. We also cover how things like scar tissue, steroid medications, and daily stress can quietly impact insulin sensitivity. If you’ve ever wondered why your numbers feel unpredictable, this episode will help you identify what to adjust before blaming your pump or algorithm.

Inside Patient #1’s Islet Transplant Without Immunosuppression
Monday Feb 23, 2026
Monday Feb 23, 2026
I speak with the first patient in a groundbreaking type 1 diabetes cure research trial using encapsulated neonatal pig islet cells—without immunosuppression. Six months after his transplant through the OPF-310 study led by Otsuka Pharmaceutical and University of Illinois Chicago, Michael Revland's insulin needs have been cut in half as the cells have begun maturing and producing insulin. We dive into how this porcine islet cell transplant works, why it differs from stem cell and donor islet trials, and the risks and rewards of pursuing insulin independence. He also shares why he declined an immunosuppression-based transplant study and the emotional highs and lows of being patient number one.

This Implantable Artificial Pancreas Could Change Diabetes Care
Monday Feb 09, 2026
Monday Feb 09, 2026
I speak with Portal Diabetes about how its next-generation implantable pump could bring this technology back—smaller, smarter, and potentially fully closed loop using CGM data. Because insulin is delivered into the intraperitoneal cavity, it acts far faster and clears the body quicker than subcutaneous insulin, which could mean fewer highs and lows and far more spontaneity. I ask them how the pump works, what the surgery and refills look like, and what real users have shared about living with an implantable pump. We even do a live demo—so if you want to see it in action, check out the video on YouTube or clips on my social channels.

Monday Feb 02, 2026
Monday Feb 02, 2026
Gary and I take on one of the most important skills in modern diabetes management: understanding and using your data. We break down how to analyze CGM and pump data to identify patterns, trends, and problem areas without getting overwhelmed by numbers. The conversation covers basal insulin, bolus timing, insulin to carb ratios, correction factors, and what metrics like time in range and glucose variability actually tell you about your settings. We also discuss when to make adjustments on your own and when to involve a diabetes educator.

This New AID Uses Biometrics to Automate Insulin - Inside Rheos
Monday Jan 26, 2026
Monday Jan 26, 2026
Using biometrics like heart rate, stress, and activity to automate insulin delivery in real time, Rheos is a next-generation AID system designed to learn and adapt directly from the body. I sit down with Jonathan Fitch, Founder of GlucoSense, to explore how this settings-free, AI-powered AID system uses wearable data to continuously adapt insulin needs throughout the day. We get into features like logging meals by photo or voice, planning workouts through an in-app LLM, and how the system dynamically adjusts dosing based on real physiological signals. Jonathan also shares how a life-threatening seizure during college pushed him to rethink diabetes automation—and how that experience shaped Rheos’ goal of dramatically reducing the daily management burden.

More about Justin
Justin was diagnosed with Type 1 Diabetes in May 2021 thanks to the diabetes community on Tik Tok.
Originally, misdiagnosed with type 2 diabetes, Justin posted videos on Tik Tok of his symptoms and what his doctor had him doing at the time - testing blood sugar, taking long acting insulin - when the diabetes community found his videos, they blew up and they told him he had type 1, needed and Endocrinologist, a cgm and a pump. They got him the help he needed and I owes them his life.
